Additional Benefits
In addition to the above items, the intended parents pay for the following items: Psychological Support Anticipated psychological counseling fees based on four months trying to conceive. Surrogate support throughout term of pregnancy, including a monthly meeting with the psychologist.
Insurance Medical Insurance Medical insurance premiums. Approximately $200 per month, 15 months. Surrogate must provide proof of insurance, including carrier name, policy information and premium amount. Life Insurance $250,000 term policy for surrogate in case of accidental death. Attorney Fees: $750 Up to $750 of Surrogate's attorney's fees will be paid for by the Intended Parents for negotiating and reviewing a surrogate agreement.
Surrogate Benefits: Surrogate Compensation $20,000 Paid to surrogate in installments over the course of the pregnancy beginning upon confirmation of pregnancy (usually 14-day blood test). In the case of multiples, the surrogate receives $5,000 for each additional baby delivered. Monthly Allowance $200/month Surrogate expense reimbursement for 15 months. In lieu of itemized costs, this fee includes reimbursements for mileage, lost wages prior to pregnancy, telephone calls, faxes, postage or federal express charges, housekeeping, childcare, and miscellaneous expenses such as pregnancy test kits, vitamins, etc. Begins at point of matching to one month after birth: $200 per month. Maternity Clothing Allowance $800 Paid after completion of first trimester. Mock Cycle Fee $250 Surrogate will take medications to simulate a cycle to test thickening of uterine lining. Paid upon completion of mock cycle. Embryo Transfer Fee $500 Paid after each embryo transfer procedure to compensate surrogate for time associated with bed rest. For Traditional Surrogates, this fee shall be paid upon completion of six (6) artificial insemination cycles. Invasive Procedures $500 Surrogate shall receive $500 per procedure for amniocentesis or other prenatal diagnostic testing, early termination procedures, fetal reduction, or ectopic pregnancy to compensate for the discomfort associated with the above procedures. C-Section $1,500 Surrogate shall receive an additional $1,500 at the time of delivery if she is required to deliver via cesarean section, to help compensate for the additional pain and suffering, added risks, and longer recovery time. Lost Wages $3,000 Estimated lost wages for surrogate (approximately 8 weeks) and spouse (three to five days). If surrogate is a stay-at-home mom or unemployed, no lost wages are paid to surrogate (lost wages for spouse still apply). Surrogate and husband must provide proof of actual lost wages.
